Healthcare Resource Utilization and Costs Among Patients With Gastroesophageal Reflux Disease, Barrett’s Esophagus, and Barrett’s Esophagus-Related Neoplasia in the United States

Background: Gastroesophageal reflux disease (GERD) is a risk factor for Barrett’s esophagus (BE) and BE-related neoplasia (BERN). Objectives: This study aimed to evaluate healthcare resource utilization (HRU) costs associated with GERD, BE, BERN in the United States. Methods: Adult patients nondysplastic BE (NDBE), (including indefinite dysplasia [IND], low-grade [LGD], high-grade [HGD] or esophageal adenocarcinoma [EAC]), were identified from large US administrative claims database, IBM Truven Health MarketScan® databases (Q1/2015-Q4/2019). Patients categorized into corresponding mutually exclusive EAC-risk/diagnosis cohorts based on most advanced stage GERD EAC using diagnosis codes medical claims. Disease-related HRU (2020 USD) calculated each cohort. Results: following cohorts: 3 310 385 172 481 NDBE, 11 516 IND, 4332 LGD, 1549 HGD, 676 EAC. annual mean number of inpatient admissions, office visits, emergency department visits by cohort 0.09, 1.45, 0.19 GERD; 0.08, 1.55, 0.10 NDBE; 0.10, 1.92, 0.13 IND; 2.05, LGD; 0.12, 2.16, 0.14 HGD; 1.43, 6.27, 0.87 total $6955 $8755 $9675 $12 241 $24 239 $146 319 Discussion: had important costs, including admissions visits. As progressed more stages, there was substantially higher disease-related utilization, being 16 times than those NDBE. Conclusions: Findings suggest need early identification high-risk individuals prior progression potentially improve clinical economic outcomes this population.
